Home Health Care Services

 

Amedisys offers a variety of home health care services to help you recover from an injury, surgery or illness or manage a chronic condition. Home health services are provided in your home, whether that’s a house, an apartment, a relative’s home or a senior living community. If you meet the eligibility requirements, in-home health services are typically covered by Medicare, Medicaid and many private insurance plans.

Your doctor will decide which home health care services are right for you. They’ll work with your Amedisys home health team to develop a plan of care that is tailored to your needs and schedule. Your team may include a nurse, home health aide, physical therapist, speech therapist, occupational therapist and/or medical social worker. Our home health professionals have in-depth knowledge, experience and training, so they can provide high-quality care to help you stay safe at home.

What services are provided by home health care? Depending on your needs and doctor’s orders, your at-home health services with Amedisys could include:

Skilled Nursing Care

A registered nurse will work with you, your doctor and the home health team to set up your plan of care. Nursing care could include pain management, monitoring, education and other home nursing services.

Physical Therapy

Learn ways to move around and do daily activities easier. Your physical therapist can help you build strength, balance and endurance. This can reduce your risk of falling or going to the hospital.

Occupational Therapy

Occupational therapists can help you learn ways to do daily tasks like eating, bathing and household chores. We can also teach you how to use special equipment and make changes in your home to reduce your risk of falling.

Speech Therapy

A speech therapist can help you communicate more clearly if you’re struggling with speech or language issues. We can also help improve your eating and swallowing abilities.

Home Health Aide

Home health aides help with personal care needs such as getting out of bed, moving around, bathing and getting dressed.

Medical Social Work

Medical social workers act as advocates for your needs and help coordinate your care with the rest of your healthcare team. They can also provide counseling and referrals to local resources in your community.

Wound Care

Home health nurses monitor and care for a variety of wounds to help reduce complications, promote healing and keep you safe at home.

Specialty Programs

Our specialized empowerment programs help you learn to manage your chronic condition to control symptoms and improve your quality of life. These include programs for heart failure, COPD, diabetes and fall reduction.

Mental Health Home Care

Mental health home care is specialized in-home care for people with mental health disorders such as depression and anxiety.

If you have an urgent need, you can reach one of our home health specialists 24 hours a day, seven days a week. With the support of home medical services, you may be able to avoid unnecessary trips to the hospital.
