Who is eligible? Any individual with a terminal illness such as heart disease, pulmonary disease, dementia, Alzheimer's, HIV/AIDS, or cancer is considered eligible for hospice care.
Amedisys' specialized team of hospice professionals works with the patient, family members, and attending physician to develop a plan of care that will best meet the patient's individual needs. The hospice care team consists of:
