In addition to skilled nursing and physical therapy, in many areas we're able to provide specialty care to our patients, like occupational therapy, speech-language pathology, psychiatric nursing, social workers, and certified wound/ostomy care tele-consulting.

Continuum of Care

We implemented the home health industry’s first nationwide care transitions program back in 2009. Today, Amedisys has more than 400 care transitions coordinators across the country helping thousands of patients each day transition safely home from acute-care settings.

Whether post-acute services, chronic care management, palliative care, or hospice care, we have many care centers across the country with integrated care teams to make this transition seamless.
